The Holly Inn: Program of Care
The Holly Inn at Brandermill Woods provides a therapeutic environment
for residents with dementia disorders. This environment is designed
as a safe and structured unit promoting independence and an individualized,
holistic plan of care for each individual. The Holly Inn
provides individualized resident care. An Individualized Service
Plan is created using the following: Uniform Assessment Instrument,
Report of Physical Examination, Cognitive Physician Assessment,
Brief Cognitive Rating Scale, facility’s own functional
assessment tool, family and resident interviews.
The Holly Inn Program of Care includes
- Residential – private, unfurnished
apartments; transportation to local medical appointments.
- Therapeutic Recreation – structured
daily program; community outings; activities that focus on resident
normalization from past experiences and lifestyle; activities
that encourage socialization, sensory stimulation and physical
exercise; activities that focus on enhancing the cognitive abilities
of the residents.
- Dining Services – three meals per
day served family style in The Holly Inn’s dining room;
snacks.
- Personal Care – medication administration
and monitoring; assistance with bathing, dressing, grooming,
toileting, transferring, eating and incontinence management;
physical, occupation and speech therapy, as needed; coordination
of medical appointments; emergency medical care – licensed
nurse available 24 hours.
- Housekeeping – bed linens changed
weekly (twin size bedding available); clean towels daily; weekly
housekeeping.
Holly Inn Admission Criteria
- Will have a physician diagnosis of a dementia disorder or
other related cognitive disorder. Will be assessed for the unit
using the Brief Cognitive Rating Scale, with a score ranging
between a 4-6.
- Will demonstrate behaviors associated with dementia such
as memory deficits; poor judgment, disorientation to time, place,
person; decreased attention span; mood fluctuations and wandering
behaviors.
- Will be able to consistently benefit from therapeutic programming
not less than 50% of the time.
- Will be ambulatory including self propelling in wheelchair,
or use assistive devices such as walkers or canes.
- Will be able to feed self with cueing.
- Will be continent of bowel and bladder with a scheduled toileting
plan.
- Will be able to benefit from verbal and visual cueing to
assist with activities of daily living.
