The Chesterfield: Program of Care
The Chesterfield at Brandermill Woods is an Adult Care Residence
serving adults that may have physical and/or mental impairments
causing them to require minimal to moderate assistance with a maximum
of four of their activities of daily living. Individuals residing
at The Chesterfield have access to our total Assisted Living Program
outlined below. The Chesterfield at Brandermill Woods provides
individualized resident care. An Individualized Service Plan is
created using the following: Uniform Assessment Instrument, Report
of Physical Examination, facility’s own functional assessment
tool, family and resident interviews.
The Chesterfield Assisted Living Program includes
- Residential – private, unfurnished
apartments; transportation to local medical appointments.
- Therapeutic Recreation – daily structured
activities; community outing twice weekly.
- Dining Services – three meals per
day served in the dining room; snacks.
- Personal Care – medication administration
and monitoring; assistance with bathing, dressing, grooming,
toileting, transferring, eating and incontinence management.
Will provide on-going supervision for safety; physical, occupational
and speech therapy as needed; assistance with arranging Home
Health Services, as necessary; coordination of medical appointments;
emergency medical care (licensed nurse available 24 hours).
- Housekeeping – bed linens changed
weekly (twin size bedding available); clean towels daily; weekly
housekeeping.
The Chesterfield offers Levels of Care programs ensuring that
residents receive the care they need and pay only for the services
they require. This allows residents to remain within the community
as needs change and provides an environment conductive to the
well being of each resident. The Assisted Living Program Director,
prior to moving in and then periodically thereafter, assesses
each resident’s level of care. This ensures that each resident’s
service plan is tailored to his or her individual needs. If there
is a significant change noted, level of care will be evaluated
within 30 days.
